Душа ушла в пятки - to have one's heart in one's mouth

(У него) душа ушла в пятки   - (He) had (his) heart in (his) mouth (= He was very scared)

 

Когда Катя поняла, что в квартире были воры, у неё душа ушла в пятки. = When Katya realized that there were burglars in the apartment, she had her heart in her mouth.

Когда я увидела змею, у меня душа ушла в пятки. = When I saw the snake, I was horrified.

 

Literally the expression means "His soul went down into his heels"!
